Evaluations & Outcomes. Program Description. The California Substance Abuse Treatment Facility (SATF) in Corcoran, California, opened in 1. Inmates who were interested in receiving treatment and who were currently serving time for an offense involving substance use were given priority for being housed in the facility. The SATF included two residential treatment units, known collectively as the Substance Abuse Program, that were separated from the rest of the prison population. While some inmates voluntarily entered treatment, others were mandated to participate. In both of the programs, treatment services were provided by private community- based contractors (one of the programs was provided by Walden House, and the other by Phoenix House). Each program housed up to 7. Despite a few minor differences between the two programs, both were based on the therapeutic community model, a holistic approach that is intended to provide a culture and environment that encourages pro- social behaviors, respect, and personal responsibility. Both programs provided 6 to 1. Program participants spent four hours per day in treatment and four hours in work or training assignments. The programs consisted of three phases: orientation, primary treatment, and a reentry planning phase. Program participants were strongly encouraged to enroll in aftercare treatment upon release. A transition plan was developed for each participant, and participants were referred to three types of aftercare programs: residential programs, sober living/halfway houses, and drug- free outpatient services.
Aftercare programs were located throughout the state, but all were licensed by the California Department of Alcohol and Drug Programs. Parole agents located at the SATF assisted with the development of the transition plans. Recommendations for Practice. The researchers suggest that a high rate of treatment staff turnover may indicate that the program had not yet reached maturity at the time that the study was conducted. They note that the program experienced difficulties in recruiting and retaining qualified staff. They also point to the large size of the program as a hindrance to the development of a therapeutic culture. They suggest that all of these factors, combined with the fact that some inmates were mandated to participate in the Substance Abuse Program and that aftercare services were diffuse and participation in such services was low, may have led to a lack of treatment effects on post- release outcomes. They speculate that the program would require about four to five years to achieve a qualified, stable group of treatment personnel and to establish a robust therapeutic culture. Accordingly, they make the following recommendations, which may also be applicable to similar therapeutic community programs. Reduce the size of the program. Avoid building treatment and correctional facilities in remote areas, as this can make it difficult to recruit and retain qualified program staff (alternatively, increase wages to entice staff to move to the area where the prison is located). Mandate or offer incentives for participating in aftercare programs (for example, by making residential aftercare participation a condition of parole). A sub- analysis comparing members of the comparison group who reported that they did not want treatment to program participants who did not want treatment but were mandated to participate yielded dramatic and statistically significant differences in recidivism outcomes: 6. These findings suggest that programs should target individuals who volunteer for treatment, implement strategies to enhance motivation for treatment, and offer incentives for participating in treatment. Suggestions for Future Research. Because little can be known from a single study, additional research is needed to determine whether the treatment programs at the California Substance Abuse Treatment Facility are effective. In particular, future research should endeavor to use random assignment to eliminate the possibility of pre- existing differences between groups. It should also be noted that, in the study reviewed here, several implementation issues may have influenced the study results; the study findings therefore may not apply to the programs as they currently operate.
